Hands-on Support

This option could be working alongside yourself or a family carer to give some “tricks of the trade” or to give you and them confidence in something that is new.

It could be providing care to allow a family carer to leave the house for whatever reason.

This option can also be useful as an additional support adjusting to an unexpected change in condition, particularly if this may result in unwanted admission to hospital. It may also support discharge from hospital.

It can be reassuring for yourself and family carers to have an experienced person in the house whilst learning about a new situation. It can also be helpful to get advice on other services that could help and assistance with completing forms.
